Other than the rebound contribution I'm not sure specifically what happened regarding the smoke and no lock break, but she may have had disloyalty or something, smoked last valerian puff, shielded, gg. A couple of other things are possible for that scenario too but just nowhere near as cool.

Tesha realized she had rebounding down as I stabbed the gecko/kalmia and tapped ss. As soon as it was able, it smoked what we all assumed to be valerian...until nothing is eaten after the epidermal line that ends up being failure.
It looks like a bizarre bug until that big ugly head of weapons rebounding appears completely out of turn.
She was a victim of poor prioritizing after the addition of smoke balance (perhaps Svo simply clocked multiple ones on a queue, with an oversight into defenses > cures).

It looks like a bug OR an illusion, which is a common one for people using anti-tracker systems (like me).
