The affliction route is trivial to pull off, and that's not even with him doing some "spin" thing to put me further behind balance. Not sure what that is, idk.
The damage route is insanely ridiculous, considering that they absolutely have the ability to stick sensi prior to skullbashing as it is.
I see a few issues on my end (not tree'ing properly, not wearing armour) though I also believe the class needs to be looked at. Classleads are closed, but these seem more like design flaws than classlead issues. Maybe that's just me though!

Current classleads should help address skullbash damage. When you were wearing armour, skullbash did 60-70% which is usual at 19 strength and a level 1 head break. You were able to survive a two limb prep double skullbash using healthtrans. While I'm sure people aren't in a place to be considering buffs to Sentinel, I'm concerned that fixing skullbash (which needs fixing) will neuter our prep path - as we have no parry bypass, a two limb prone is all we can reliably achieve. With reasonable skullbash damage, this is very unlikely (impossible if they can prone heal) to get someone low enough for the transfix->rattle->bind->dismember path to be viable.
Spinning is a defense which costs balance and high wp drain and is taken down when the sentinel attacks. While up, it's like a 100% heartsfury and is good for stacking venoms on someone immediately before opening with a doublestrike at the start of a fight.
Re petrify, I think raising the threshold to 5 has merit (wasn't it 5 originally?) though I think if that happens a) the pool of affs should be widened to include stupidity or b) another candidate aff such as reckless or epilepsy should be added to pet affs.
Also, as a BM with shindo blind/deaf (assuming it's like kaido), I think you should be immune to sensitive skullbashes and dizzy+petrify as you can keep blind and deaf up balance free.
I find sentinel alright to handle as shield, rebounding and fly totally wrecks their attempt to petrify. Smart prio changes to avoid the petrify does wonders too. Sure it's deadly, but I haven't found it too scary since i fixed my prios.
You really do need to have unique prios for sent or you're screwed. Skullbash is badly tuned, though, it's overpowered vs bad armor and useless vs high.
If impatience and one mental, the prio swap impatience so you can focus it away I believe is the prio swap you're referring to?
You have to keep in mind that they can triple afflict 2 out of every 3 rounds (if memory serves that's the progression in the fights) so you're far outpacing herb balance and can reafflict impatience pretty easily unless I'm mistaken.

Why didn't the head fix in that log? According to your timestamps, 4.2 seconds passed since the apply. Unless you gagged the head healing and that's Skullbash's base damage. (wtf!)
But it's honestly not the damage that seems most ridiculous, it's the speed that gets me. Disembowel can one-shot you in the same manner, but even the fastest DSB setup takes 5.5 seconds, enough time time to tumble out and avoid death. That Skullbash was 4.4 seconds from head break to dead. Atalkez had a window to shield/run, but if Lyrin had timed it better, Atalkez wouldn't have had any opportunity (being off balance) to avoid that. That's kinda nuts.

Nah I don't gag it. I've noticed small inconsistencies in herb and salve timing with server curing. 4s rest really being 4.03 or herb balance being 1.62 instead of 1.6. Have died tons of times to milisecond difference when something should have been finished/available and it wasn't.

Rive is 2.25 seconds flat to break shield. Rivestrike (rive + venom) is just over 2.3 seconds with a level 3 spear. You can also break shield with lemming, which comes at the cost of using a pet affliction that round.
Something looks a bit odd with the timestamps in Atalkez's log - it's always possible to land a skullbash before broken head cures with a level 3 axethrow (~1.4sec) -> trip (2.475 sec) -> skullbash. It isn't possible with a forged axe, however.

Sentinels can only give impatience through the slow spear attacks, which gets you some breathing room (3s balance pre nimble unartied)
