ETA: Even if it was right after, it still strikes me as more of an "Oops" and an incident of miscommunication. Considering that you were thinking of a duel in the first place, is it really a big deal?

I was being IC. We agreed to spar, so we went to delos, I challenge her just before she challenged me. By show of hands, who would have caught that?
I wouldn't have caught it, but I would have considered me not catching it a purely OOC thing. It's only on an OOC level that it's something you can miss so easily. IC, she was all like "I challenge you to a duel to the death! Do you accept?", with no indication that she was mumbling the "to the death" part.
I'd IDEA to make that line stand out more, or to require an additional argument for accepting a duel to the death like "ACCEPT <player> TO THE DEATH"
It seems like it was an honest mistake rather than some underhanded scheme to trick you into fighting to the death. Besides, I'm not really sure what the point of the regular spar would have been other than for you to see if you could win against her before committing to actually losing anything.
Zinka dueling to the death was weird, and her defending it after a simple apology was unnecessary, but nothing "dishonorable" took place. It's less Knightly for you to complain about a death than it is for her to make a mistake.

Edit: Well, the AB file says half. The few times I used starburst I definitely felt like it was full though. Which is the main reason I don't use starburst anymore.
