So, I found this out one day when Niz undertook a penance to starve himself to death. It is impossible to starve to death in Achaea. The lowest you can drop to is to 1 health.
@Katzchen: Granted, Niz decided to walk back to Mhaldor from the Ivory Temple at the completion of his penance, and got looked at the wrong way by a lycopod and died, but that's immaterial.

According to Clementius, you can die from starvation, even if it's very unlikely. He didn't give specifics though.
I've been logged in for more than 24 hours while starving before, without any indication that I might die (or noticeable endurance loss).

I got hoisted out of Ashtan by Proficy once (stupid me walking around without mass up during that war with Mhaldor). He drops me in front of Mhaldor's entrance, with Cahin standing there.
My escape plan was "goto Ashtan."
I walked right out of a room with Proficy attacking and Cahin standing by to help out, walk right past Aovul, and the only thing that hinders me before I get back to Ashtan is a lycopod webbing me.
I got killed in one hit on an adventure near Mhaldor and had a journal with 17 paintings in it get nabbed by an ormyrr the first time I went near Mhaldor. I never returned even though I got the journal back. Mhaldor is really creepy.

This has now been verified. Once you're down to 1 HP, you lose endurance instead of health. And if the hunger damage tick brings you to 0 endurance, you die. The amount of endurance you lose is pretty random, seems like it can be anywhere from 10 to 200. Most of the time, natural regen at inept fitness is more than enough to keep your endurance at full, and you'll never die. But if you have other things draining your endurance or you're already nearly out, it can kill you (or if you just get extremely, ridiculously unlucky it could slowly bring you down to 0 from full endurance).
You double over in pain as your stomach virtually collapses. The agony of the cramps becomes too much for you to bear, and your badly under-nourished body finally gives in as you slip into the deepest of all sleeps.

Someone told me you can't actually die from starving back in Hashan, when the same thing happened. Cannot verify this is true though.
Also, shouldn't you be y'know, killing her if she's afk, not saving!
